What is a Veterinary Information Form? 

We never want to guess with your pet's health and so we need a history from your primary veterinarian prior to the first adjustment. This is a PDF that you can forward over to your vet. It opens the doors of communication for Cloudberry Lane and your veterinarian to collaborate on care. We believe that every pet should have a team supporting their journey to health and wellness. 

Most veterinarians do not require you to make an appointment with them for this signature if they have seen your pet within the last year. This is dependent on other factors and your veterinarian will let you know if your pet needs to be seen. Most practice members get this signature back to us by the second appointment. If it is signed before that, we will adjust on the first appointment after we perform a gait analysis to establish the baseline of health for your animal.

Scheduling

Yes, we are taking new patients! Your first appointment is a new patient consultation where we will sit down with you and your pet to delve into their health concerns. We will perform a chiropractic examination and an electronic gait analysis to get to the root cause of their symptoms and determine if there are any high-risk areas that may become injured in the future.
